Before you get started, let’s do a quick inventory!  


Your Diabetes Test Service should contain:  

  • A Test Request Form (TRF) - you’ll need to fill this out so we can complete the blood sample analysis.
  • A sample collection pack accompanied with step-by-step instructions. 
  • A pre-paid postage envelope to mail your sample to our certified Australian pathology laboratory.   

Make sure you’ve got a pen handy, and then you’re ready to go!  


First up, take a read through the step-by-step instructions. 


Finished reading? Good work! Now it’s time to get prepped. 


Wash your hands with warm water for at least 30 seconds and dry them with a clean towel.  


This will avoid unwanted contamination of your sample and sample card. 

 

Next, let’s get these forms filled out.  


On the blood collection card, you’ll need to write:  


•    Your full name  

•    Date of birth   

•    Date that you completed the test 


 


Now, complete your Test Request Form. There’s a bit more info to fill in here, but it won’t take you too long!


 


Next, let’s take the sample. 


Follow the step-by-step instructions for taking your dried blood spot test.  


Place one drop of blood on each of the required circles – making sure the drops are large enough to fill out the entire circle and to soak through to the back of the card. 


Seal your sample. 


Once you’ve completed your test, put your sample collection card into the small card envelope, then place this inside your prepaid envelope.  


Review your Test Request Form  


It’s important all the necessary details have been provided and are correct. Pay special attention on the date and signature on the bottom of the form. Without a signature we are not authorised to process your sample and it automatically becomes invalid. If you want to retake the test, you will have to purchase a second test service to collect another sample.


Post it!  


Now that you’ve checked that your Test Request Form has been completed, dated and signed, we’re ready to go! Enclose the form in the prepaid envelope with your sample and seal it. Post this envelope within 24 hours.
